Approval Protocol

Partner and Co-brand

This disclaimer must appear on all platform surfaces and marketing materials where health outcomes are discussed.

Confidentiality

The investor deck, financial projections, and any materials marked Confidential are for the sole use of the intended recipient. Reproduction or distribution without written permission is prohibited.

Trademark and IP

LONGEVITY PASSPORT wordmark, brand colors, visual system, and associated intellectual property are the property of Longevity Passport. Unauthorized use is prohibited. The brand identity system defined in this document, including the coordinate crosshair motif, color palette, and typographic system, constitutes trade dress.

Provider Liability

Longevity Passport verifies provider credentials and protocols at onboarding and annually thereafter. Platform listing does not constitute an endorsement of any specific clinical outcome. Users engage with providers at their own discretion.

Data and Privacy

Health data collected on the platform is subject to HIPAA where applicable and applicable local privacy law. Patient-owned data model: user records belong to the user and may be exported at any time. FHIR R4 compliance planned for Phase 2 via Medplum.
